Care Delivery
Following a request for care by either a referring agency or private individual we will arrange to meet Service Users in their own home to discuss their assessed needs and to complete a Care Plan (see below). We always aim to keep our Service User fully informed at all times and this includes providing the name of the carer and the expected date and time of first and subsequent visits.
The Care Plan Pack
The Extra Hands Care Plan Pack provides a complete guide for both the carer and service user. For the duration of service provision the pack remains at the Service Users home and is reviewed annually or more frequently if required. Each and every Care Plan includes:
- Comprehensive risk assessment
- Emergency contact numbers and procedures
- Accident / incident report sheets
- Moving/handling & transfer assessment
- Moving/handling & transfer procedure
- Service User general information (ie allergies, sleep patterns, mobility etc.)
- Daily routine information
- Duties record (daily entries detailing carer duties performed)
- Record of financial transactions
- Medication administration record
- Carer notes
- Prescription & medication instructions
